No, at your initial session, Dr. Simpson will determine what lab work she suggests you discuss with your health care providers.
If you've had bone density testing, request the correct report pages from your imaging facility (NOT your doctor) using this form.
Please only mail in one (1) package -- do not fax or scan -- past 3-5 years of lab work (blood work, hormones and bone-related tests only). Bone density test results via CD are acceptable, but please include a self-addressed, stamped envelope so we can return these to you.
We accept credit cards (Visa and Mastercard). Pay online through our secure encryption system at time of appointment schedule, or by phone. We strongly prefer online payment, but if absolutely necessary, we can accept a check.
New clients are emailed a link to book and pay for their appointment -- only after all records and forms have been completed and received. New clients can also mail a check to secure an appointment slot.
Ongoing clients already in our system can pay online when selecting the follow-up appointment type or email audio response.
